Please note that from the 20 July 2010 the Criminal Records Bureau (CRB) is no longer accepting the original style white CRB application forms with black typed question boxes.
The new style forms are different from the originals in several respects:
CRB guide notes are written on the front cover - the outer covers now having a pink background.
The Independent Safeguarding Authority (ISA) Logo is also present alongside the CRB Logo on the front cover.
Applicants now complete the second and third pages of the form. Mandatory boxes are blocked in yellow with purple boxes.
Agreed Verifiers continue to check the completed forms and fill in relevant boxes in sections A and B as well as their own sections on the back page in Sections W and X.
The new CRB application form was designed to allow members to apply for a CRB check and Independent Safeguarding Authority (ISA) registration on one simple form. However, as a result of the Government's recent announcement to halt the launch of the ISA registration phase of the new Vetting and Barring Scheme this form should only be used to apply for a CRB check.
The new style CRB forms still have to be returned to the Archery GB CPO in the Operations Office based at Lilleshall National Sports Centre to be countersigned prior to being progressed on to the CRB.
If you still possess any original application forms, please destroy them by shredding and apply for the new style CRB (ISA) Application Form from Archery GB via the Operations Office at Lilleshall: telephone 01952 602 792 or e-mail anne.rook@archerygb.org
Please note any old style completed CRB forms received will be returned to the applicant with a new style form, as the CRB will no longer accept or process the original forms.

The CRB has published a document to help you and your verifier complete the new style form. More
The CRB offers a confidential checking process to protect the privacy of applicants who have undergone gender reassignment. This process provides applicants with the opportunity not to reveal their previous name(s) an/or gender to their employer, or other organisation, when applying for a CRB check.
The CRB is fully committed to promoting equality and eliminating discrimination on grounds of gender identity and this process ensures that a transgender person's application is dealt with sensitively and without the need to disclose previous names to the employer, whilst maintaining the integrity of the CRB service.
The CRB provides a designated point of contact for transgender applicants, dedicating a small resource to deal with any queries that are raised, and provides help and support throughout the process. They are also solely responsible for contacting the applicant if further information is required.
If an applicant has a conviction under their previous details, it will show on a completed check. If a certificate is going to show any personal details that the applicant may not wish to disclose, the applicant will be informed of this and they will have the opportunity to withdraw the application.
Anyone wishing to use the process must contact the CRB Sensitive line directly on 0151 676 1452. Alternatively the CRB has a dedicated e-mail address for enquiries regarding transgender applications: crbsensitive@crb.gsi.gov.uk
