CRB Disclosure Applications

Please note that from the 20 July 2010  the Criminal Records Bureau (CRB) is no longer accepting the original style white CRB application forms with black typed question boxes.

The new style forms are different from the originals in several respects:

CRB guide notes are written on the front cover – the outer covers now having a pink background.

The Independent Safeguarding Authority (ISA) Logo is also present alongside the CRB Logo on the front cover.

Applicants now complete the second and third pages of the form. Mandatory boxes are blocked in yellow with purple boxes.

Agreed Verifiers continue to check the completed forms and fill in relevant boxes in sections A and B as well as their own sections on the back page in Sections W and X.
 
The new CRB application form was designed to allow members to apply for a CRB check and Independent Safeguarding Authority (ISA) registration on one simple form.  However, as a result of the Government's recent announcement to halt the launch of the ISA registration phase of the new Vetting and Barring Scheme this form should only be used to apply for a CRB check.

The new style CRB forms still have to be returned to the Archery GB  CPO in the Operations Office based at Lilleshall  National Sports Centre to be countersigned prior to being progressed on to the CRB.

Please note any old style completed CRB forms received will be returned to the applicant with a new style form, as the CRB will no longer accept or process the original forms.
